Purpose of this Questionnaire

This Questionnaire is for people for whom Richard Keyt prepared an estate plan. Richard's clients to use this Questionnaire to tell Richard the changes to make to their estate plans. At the end of the Questionnaire you can request a phone or an office consultation if you have questions.

Legal Fee for Our Update Services*

$1,247 for one person or $1,497 for two people to update your living Trust, Certification of Trust, Last Will & Testament, Living Will, Financial Power of Attorney, Healthcare Power of Attorney, HIPAA Authorization, and Assignment of Personal Property. We update all of your estate planning documents to include changes you want to make for your different agents, successor trustees and beneficiaries. We also update your documents to include changes we made have made to our documents since you signed your estate plan. The fee includes a conference to sign your documents, scan them and give you digital pdf copies on a thumb drive.

You only need one successor trust, but we recommend you have at least two in case yo;ur first choice dies before you. Personal Representative of Your Estate

If you die and a probate is needed, somebody will have to file a petition with the probate court and ask the court to appoint him or her as the personal representative (aka executor in some states) of your estate. The personal representative is an important position because the PR will have the legal power to collect your assets, pay your last expenses and the legal obligation to distribute your probate assets to your heirs named in your Last Will & Testament.

You must name at least one person. You can name more than one person, but you must list them in the order of priority in which you would like them to serve. People named as personal representative in your Will will have priority to get the appointment, but ultimately the court will decide who will actually be the PR.
